Introduction
When undergoing minor skin surgery in Japan — such as removal of an epidermoid cyst (atheroma), lipoma, or mole — costs are determined primarily by three factors: whether Japan’s public health insurance applies, your cost-sharing ratio, and the location and size of the lesion. This article outlines the estimated costs at IC Clinic Tokyo, broken down by insurance-covered and self-pay treatments.
To summarize upfront: for patients with insurance at the standard 30% co-payment rate, epidermoid cyst surgery — including consultation, tests, the procedure itself, and pathological examination — typically falls within the range of ¥10,000–¥15,000. Patients without insurance coverage pay the full amount (100%) for the same services. All figures below are estimates; the final cost will be confirmed after examination by a physician.
How Costs Are Determined Under Japan’s Insurance System
Fees for insurance-covered treatment in Japan are calculated using a nationally standardized medical fee schedule. Patients pay only a portion of this amount at the clinic window — the co-payment rate varies by age and income, but is generally 30% for working-age adults. At our clinic, eligibility is verified using a My Number Card (with insurance functions enabled) or an Eligibility Confirmation Certificate.
Eligibility for Japan’s public health insurance is not determined solely by visa status; it also depends on whether enrollment procedures have been completed and the type of employment arrangement involved. Patients who are not enrolled — including those on short-term stays — are treated as self-pay patients and will be charged the full 100% of the standard insurance-based medical fee.
Please note that insurance coverage is determined by a physician at the time of examination, based on symptoms and medical necessity. Treatments that are purely cosmetic in nature are not covered by insurance and will be billed as self-pay.
Estimated Costs Under Insurance Coverage (30% Co-Payment)
In addition to the surgical fee for epidermoid cyst or mole removal, separate charges typically apply: approximately ¥1,000 for consultation and prescription fees, approximately ¥1,000 for examination fees, and approximately ¥3,000 for pathological examination. Pathological examination is performed as a general rule, based on the physician’s judgment, given that malignant tumors have been reported to arise from certain growths.
Estimated Costs Without Insurance Coverage (Self-Pay, Tax Included)
Treatments for cosmetic purposes, or for patients who do not have Japanese health insurance enrollment, are handled as self-pay. The following are examples of self-pay menu options at our clinic (tax included, standard pricing):
- Earlobe repair (when not covered by insurance): ¥55,000. Piercing hole correction: ¥33,000–¥88,000 depending on size
- CO₂ laser for moles and warts: Mole laser trial (face only) ¥3,300; Skin tag laser trial ¥5,500
- miraDry (underarm odor / hyperhidrosis): Single session Standard Plan from ¥169,500 (anesthesia and procedure fees included)
Patients without Japanese health insurance who undergo surgery such as epidermoid cyst removal will be charged the full 100% of the standard insurance-based fee for the same services. An estimate will be provided prior to treatment.
Items Not Included / Potential Additional Charges
- Initial or follow-up consultation fees, prescription fees, examination fees (such as ultrasound), and pathological examination fees (the “separate” charges noted above)
- MRI examination for lipomas (performed at an affiliated medical facility; billed separately from surgical fees at our clinic)
- Medical certificate: ¥5,500 per document (tax included); Fee statement: ¥3,300 per document (tax included). Both are processed by mail. Medical certificates can also be issued in English.
- If you forget your insurance card (My Number Card or Eligibility Confirmation Certificate): You will be asked to pay the full amount on the day of your visit. If you bring the original card and receipt to our clinic within the same calendar month, we can process a reimbursement at that time. For requests from the following month onward, please contact your insurer directly to arrange a refund.
